A medical care coordination network

The cases you refer out
are the ones nobody
is equipped to take.

Patients heading into major medical treatment need a dentist who works to a treatment date and can coordinate with the physician running their care. Most practices are not set up for it, so the patient bounces. Guident is the network that makes you the practice that can.

The cases

One kind of patient, and one thing in common.

Every one of them is under someone else's care before they are under yours, and every one of them is on a clock you do not control. That is exactly why they are hard to take, and exactly why so few practices take them.

A treatment date already set
Clearance is gated by a date the medical team has already set, and the workup ahead of it is unforgiving.
An unresolved source of infection
It can be a reason to postpone the medical treatment, and postponing is not a scheduling matter for the patient or the physician.
A window measured in days
Everything that has to happen in the mouth must happen before treatment begins. Afterward, the same work carries a different and much longer risk.
A schedule full of other appointments
The patient already has a calendar that every other appointment has to fit around, including yours.
These are not difficult dental patients. They are medical patients whose care is held up in the mouth, and the practice that can take them is the one the hospital calls next time.
What membership includes

You do the clinical work. We do everything around it.

The reason these cases do not come to most practices is not clinical skill. It is that nobody wants to own the coordination, the documentation, and the timeline. That is the part we take.

01

The relationships, already built

You are placed with programs and physicians who already work with the network. You are not cold-calling a hospital coordinator hoping to be added to a list.

02

The coordination, handled

The exchange with the treating physician, the documentation their record needs and yours, and the timeline. Run by our team, not added to your front desk.

03

The back office, taught and supported

Your team is trained on how these cases are handled end to end, and has someone to call when a case is unusual. Nobody is left to work it out from a manual.

And the part most practices miss

The patient chooses, from everyone who qualifies

  • Every practice meeting the published qualification is shown. No caps, no territories, no waitlists, and no fee for placement
  • A physician always knows who their dentist is, because the patient picked them and the choice is recorded on the referral
  • Being the practice that answers is what earns the next case. That is a position you hold by being reachable and fast, not one we sell you
Free for physicians and hospitals

Referring a patient rather than treating one?

There is no fee for physicians, hospitals, or their coordinators. Ever. You send us the patient and the treatment date, and we handle the rest.

For physicians and hospitals →
Membership

One flat fee. It does not move with how many patients you see.

Priced as membership in the network, not as a cut of anything. What you pay is the same whether you receive two cases this year or twenty, and that is deliberate.

Annual
$10,000
per year, paid up front

The standard membership. One practice, twelve months, flat.

Three payments
$3,600
×3 over the first year

Same membership, spread across three payments. Slightly more in total for the flexibility.

Monthly
$1,000
per month

For practices that would rather not commit a year up front. The most flexible option and the highest annual total.

  • The fee is flat and agreed in writing before anything begins
  • It does not vary with the number or the value of the patients that come your way, in either direction. No per-case fee and no percentage of anything
  • Any practice meeting the published qualification joins. No caps, no territories, no waitlists
  • We verify that a credential exists and is current. We never assess skill and we never rank members
  • Physicians, hospitals, and coordinators pay nothing, at any point
Stated plainly

What we are not.

Nobody is paid for a referral. Not a physician, not a coordinator, not a member practice. This is a deliberate structure and it is reviewed by healthcare counsel. Membership is a flat fee paid by the practice that treats the patient. The people who send patients are never paid, never discounted, and never charged.
The patient is told how the network works. When a patient is placed with a member practice, that relationship is disclosed to them in writing and recorded. California requires it, it is right regardless, and we do it everywhere.
We never promise case volume, and we never project what you will collect. What you are joining is a network and a coordination team, not a guaranteed number of patients. Anyone quoting you a figure before seeing your area and your practice is selling something we would not buy.
We do not make decisions about anyone's care. The treating physician runs the case and you exercise your own clinical judgment. We coordinate, we document, and we hold the timeline.

Tell us where you are and what you already treat. We will tell you whether your area is open, and if it is not, we will say so rather than keep you in a pipeline.
